Eroxon®
Topical ED gel

Eroxon® is a topical gel applied directly to the penis — FDA-cleared in 2023 and available over the counter without a prescription. It works through a novel evaporative cooling mechanism to stimulate nitric oxide release and improve blood flow. Fast onset (around 10 minutes), no systemic absorption, and no drug interactions.

Eroxon® is genuinely different from oral ED medications — not just in formulation but in how it works. Its lack of systemic absorption makes it particularly appealing for men who can’t take oral PDE5 inhibitors due to cardiovascular medications or nitrate use — though that should always be discussed with a physician.

How it works

A genuinely different mechanism

Eroxon® works through evaporative cooling — the gel is applied to the glans (head) of the penis, and as it evaporates, the rapid cooling stimulates thermosensitive nerve endings that trigger nitric oxide release locally. Nitric oxide then activates the same downstream pathway as PDE5 inhibitors — causing smooth muscle relaxation and increased blood flow into the penis.

The key difference from oral medications: Eroxon® acts locally at the application site and is not absorbed systemically. This means it does not have the cardiovascular effects, drug interactions, or systemic side effects associated with oral PDE5 inhibitors.

The active ingredient is 0.4% nitroglycerin — a low-dose topical application of a substance that has been used in medicine for over a century. Topical nitroglycerin for ED has been studied for years; Eroxon® represents the FDA-cleared formulation of this approach.

FDA clearance vs. approval

Eroxon® received FDA 510(k) clearance in 2023 — not Premarket Approval (PMA). As with other cleared devices, this indicates substantial equivalence to a predicate device rather than rigorous large-scale clinical trial evidence. Available clinical data is encouraging, and the OTC designation reflects the FDA’s assessment of its safety profile.

Important — nitrate interaction still possible

Even though Eroxon® is topical and low-dose, it contains nitroglycerin — a nitrate. Men who take systemic nitrates (nitroglycerin pills, nitrate patches, isosorbide) should discuss Eroxon® use with their physician before using it. The systemic absorption is very low, but caution is warranted.

Honest assessment

Advantages and limitations

Eroxon® fills a specific niche — it’s genuinely useful for some men and genuinely limited for others.

Advantages

No prescription required — available OTC

No systemic absorption — no cardiovascular side effects

Fast onset — approximately 10 minutes vs. 30–60 for oral meds

No food interaction — can be used any time regardless of meals

No headache, flushing, or muscle aches like oral PDE5 inhibitors

Suitable for men with mild to moderate ED who prefer avoiding systemic medications

Limitations

Less potent than oral PDE5 inhibitors for moderate to severe ED

Shorter duration — 1–2 hours vs. 4–6 for sildenafil

Requires direct application — less discreet than taking a pill

Less clinical evidence than oral PDE5 inhibitors with decades of data

Partner may experience headache from transferred nitroglycerin if contact occurs

Still requires arousal — does not cause erections independently

Common questions

Frequently asked questions

Can Eroxon® be used with oral ED medications? +

This should be discussed with a physician before combining. While Eroxon® has minimal systemic absorption, it contains nitroglycerin — and combining topical nitroglycerin with oral PDE5 inhibitors theoretically adds to the blood pressure-lowering effect of both. This combination has not been extensively studied. Do not assume it is safe to combine without medical guidance.

Will my partner be affected by Eroxon®? +

There is some potential for partner headache from skin-to-skin transfer of nitroglycerin. Clinical data shows this is possible — the prescribing guidance recommends applying the gel and allowing it to absorb for a few minutes before sexual contact. Partners who take nitrates should be especially careful. If partner headache occurs, the gel should be washed off before contact.

Who is Eroxon® most appropriate for? +

Men with mild to moderate ED who prefer avoiding systemic medication, who want a faster onset than oral PDE5 inhibitors, or who have reasons to avoid oral medications (certain cardiovascular medications where oral PDE5 inhibitors are contraindicated — though always check with a physician). It is not appropriate as the primary treatment for moderate to severe ED where oral medications or Trimix are more effective options.

How is it used? +

Eroxon® comes in single-dose tubes. Apply a small amount to the glans (head) of the penis approximately 5–10 minutes before sexual activity. Gently massage in and allow to absorb. Do not use more than one tube per 24 hours. Wash hands after application. Allow the gel to absorb before skin-to-skin partner contact to minimize transfer.
